Insurance
Majercik Physical Therapy is contracted with most all insurance networks. If you have any questions as to your coverage prior to your evaluation, please contact us at 815-337-7410 and we will gladly check your physical therapy benefits and review them with you.
Work Related Injury?
Usually, with a documented work injury, a claim number is assigned prior to your initial visit. We obtain authorization needed for your evaluation and subsequent physical therapy. We also communicate with your case manager with regards to your rehabilitation with daily and progress notes. Local employers appreciate the way we assist their injured workers with getting back to work in a timely manner.
Medicare
We accept patients with Medicare insurance benefits. If a Medicare patient has received any kind of home health assistance just before starting therapy, we ask that they contact the home health agency to inform them of their start with out patient therapy. This is important for Medicare in order to process physical therapy claims with us in an out patient setting.
No Insurance?
If you do not have insurance, or have a very high deductible, we can discuss your options based on your current circumstance.
Payment methods
We expect co-payments at the time of service or on a weekly basis. With most insurance companies, we can estimate your co-insurance amount in advance for you. These payments are expected on a regular basis as well. For your convenience, we accept Visa and MasterCard. We also generate a statement of your account per your request. It is our goal, while you are in physical therapy, that finances do not inhibit your recovery. We do what we can to work with you should a financial situation arise. Again, communication is important on your part.
